BACKGROUND: The aim of the study is to measure the facial soft tissue thicknesses (STTs) in Bulgarians, to evaluate the relation of the STTs to the nutritional status, sex and bilateral asymmetry, and to examine the correlations between the separate STTs as well as between the STTs and body weight, height, and body mass index (BMI). In the present study, the facial STTs were measured on computed tomography scans of the head of Bulgarian adults. MATERIALS AND METHODS: The STTs were measured at 7 midline and 9 bilateral landmarks. The measurements were performed in the free software InVesalius in the axial and sagittal planes. The mean, standard deviation, minimum and maximum values, median and coefficient of variation were reported for the STT at each landmark according to the sex and BMI category. The BMI, sex and bilateral differences were assessed for statistical significance. Pearson correlation analysis was applied to assess the strength and direction of the relationships between the STTs and body height, weight and BMI, as well as between separate STTs. RESULTS AND CONCLUSIONS: The facial soft tissues in Bulgarian adults changed in accordance with the nutritional status of the individual and in both sexes all STTs augmented with the increasing BMI. For both normal and overweight BMI categories, males had more soft tissue at the majority of facial points than females, as the only exceptions were observed in the cheek zone, where STTs were thicker in females. Significant bilateral differences were observed in either sex and BMI category. Stronger correlations were established for the STTs in the jaw region and between the cheek and jaw soft tissues. Besides, the correlations between the homologous bilateral landmarks were among the strongest ones.

The therapeutic efficacy of regular physical exercises in an animal model of epilepsy and depression comorbidity has been confirmed previously. In the present study, we examined the effects of endurance training on susceptibility to kainate (KA)-induced status epilepticus (SE), behavioral changes and neuronal damage in spontaneously hypertensive rats (SHRs). Male SHRs were randomly divided into two groups. One group was exercised on a treadmill with submaximal loading for four weeks and the other group was sedentary. Immediately after the training period, SE was evoked in half of the sedentary and trained rats by KA, while the other half of the two groups received saline. Basal systolic (SP), diastolic (DP) and mean arterial pressure (MAP) of all rats were measured at the beginning and at the end of the training period. Anxiety, memory and depression-like behaviour were evaluated a month after SE. The release of 5-HT in the hippocampus was measured using a liquid scintillation method and neuronal damage was analyzed by hematoxylin and eosin staining. SP and MAP of exercised SHRs decreased in comparison with the initial values. The increased resistance of SHRs to KA-induced SE was accompanied by an elongated latent seizure-free period, improved object recognition memory and antidepressant effect after the training program. While the anticonvulsant and positive behavioral effects of endurance training were accompanied by an increase of 5-HT release in the hippocampus, it did not exert neuroprotective activity. Our results indicate that prior exercise is an effective means to attenuate KA-induced seizures and comorbid behavioral changes in a model of hypertension and epilepsy suggesting a potential influence of hippocampal 5-HT on a comorbid depression. However, this beneficial impact does not prevent the development of epilepsy and concomitant brain damage.

UNLABELLED: Many years the ß-mimetics (Partussisten, Gynipral) were first line tocolytic agents in terms of delaying premature delivery in our country. As these medicaments have been withdrawn from the pharmacy network, some concerns began to appear among our colleges considering the administration of other tocolytics and their effectiveness. Our goal is to compare the efficacy of other tocolytics with ß-mimetics. That is why we reviewed the structure of premature deliveries in the University Hospital "Stoyan Kirkovich", Stara Zagora for two periods: 01.01.2013 - 31.12. 2013 and 01.01.2014 - 01.06.2014. RESULTS: 45 of 326 pregnancies (13.8%) > 20 weeks of gestation for 2013, treated with Gynipral have ended with a premature delivery vs. 13 of 110 pregnancies (11.8%), treated with magnesium (Mg SO4, Magnerich, Magnerot), calcium channel blockers and spasmolyticsfor the period - 01:01.2014 - 01.06.2014. CONCLUSION: The results show, that at this stage, several months after the cessation of use of ß-mimetics, there is no rise of number and percentage of premature deliveries, but there is even a tendence of decline. Larger periods and greater number of cases are needed to formulate conclusions with greater significance.

The squamous part of the occipital bone is a place of many different variations. They are a result of faulty ossification in the occipital squama or due to the presence of sutural bones in the lambda region. As their differentiation is intricate because of the various criteria used, the issue of their recognition in the adult skull still remains difficult even though they can be clearly distinguished at a younger age. The aim of the present study was to compare the frequency of interparietal, preinterparietal and sutural bones in the lambda region in medieval male and female cranial series as well as between medieval and contemporary male series from Bulgaria. We also discuss the development of the occipital squama in order to set clearer criteria for further differentiation of such variations in the adult skull. In the reviewed 3 cranial series, the variations in the squamous portion of the occipital bone were observed with a low frequency. The incidence of preinterparietal bones was more common than the interparietal ones. The sutural bones in the lambda region were numerous in the series. No statistically significant sex or intergroup differences were established. So even if these anatomical variations are relatively rare, the understanding of them is of significance for many disciplines like anthropology, comparative and developmental anatomy, clinical and forensic medicine.

UNLABELLED: The goal of the research is to discover and analyze the dependence of survival on the tumor size in a cohort of 215 female patients with early epithelial ovarian cancer fallowed up for 13 year period (2000-2012). After multiple analyze among 18 other prognostic factors, using Cox proportional regression, tumor size remained one of five most significant. CONCLUSION: The bigger the size of the tumor, the poorer the prognosis for the overall survival (p = 0.0001) and progression free survival (p = 0.4) of patient with epithelial ovarian cancer at early stage.

UNLABELLED: Review of modern opinions and tendencies, concerning the role of laparoscopic surgery for the staging and treatment of ovarian cancer stage I-II. CONCLUSION: The laparoscopic approach of surgical staging of early ovarian cancer is equal and even has more advantages then conventional laparotomy if it is performed from gynecologists- oncologists with enough skills and experience in laparoscopic surgery.

RESUMO

OBJECTIVE: The authors present and analyze the modern concepts of the role of surgery as a prognostic factor for patients with early epithelial ovarian cancer (st. I-II). They review, synthetize and summarize the information from 15 studies, published in the recent years, and concerning the subject. CONCLUSION: Surgery treatment for early epithelial ovarian cancer is one of the most important prognostic factors. The appropriate surgical performance contributes either for complete staging or for optimal primary cytoreduction and strongly influence the development and outcome of disease.

UNLABELLED: The authors present and analyze the modem concepts of the role of adjuvant chemotherapy as a prognostic factor for patients with early epithelial ovarian cancer (st. I-II). They review, synthetize and summarize the information from studies and researches, published in the recent years, and concerning the subject. CONCLUSION: Adjuvant chemotherapy for early epithelial ovarian cancer improves overall survival of patients having tumors with grade 2 or 3, stagelC or grater and suboptimal surgical staging.

RESUMO

UNLABELLED: A cohort of 215 female patients with early epithelial ovarian cancer, researched for 13 year period (2000-2012). The authors are setting a goal to investigate how does the amount of chemotherapeutic medicaments, administered as adjuvant therapy influences the survival of these patients. CONCLUSION: The application of three or more chemotherapeutic agents does not improve the survival of female patients with early epithelial ovarian cancer, indicated for chemotherapy.

RESUMO

The authors present a case of a 35 years old female patient with spinocellular carcinoma of the cervix, diagnosed after byopsy and treated with radiotherapy 30 Gray, combined with Cisplatin 50 mg. per square meter, per week, 6 months before radical histerectomy and lymphonodulectomy was performed. The postoperative histology shows only traces of dysplastic epithelia, which proves preoperative therapy effective.

RESUMO

43 years old female patients endometrial biopsy diagnosis shows highly differentiated adenocarcinoma. Because of this result she underwent total hysterectomy two weeks later, and the new hystology result is: highly differentiated endometroid ovarian carcinoma.

RESUMO

Painfull myoma utery in a patient with poor health condition and many disfunctional organs: Diabetes mellitus, Chronic renal insufficiency Gr. III (most probably a result of compression of the ureters causing bilateral hydronephrosis), arterial hypertony, anemia. The authors introduce their therapeutic model and manage to perform successfull operation after stabilizing the patient and intensive postoperative care. They point out the team work and good synchronization between the hospital divisions as crucial for the success of the treatment.
